I hate to be that guy, but I'll be that guy.  The care is consistent but honestly there are a ton of conditions (eg, diabetes induced lesions comes to mind) that are vastly more profitable and lead to a long term influx of money.  Additionally, the surgical procedure is very difficult and specialized so on a profit per FTE hour it's not nearly as profitable as many.  Many cosmetic procedures are vastly more profitable to the hospitals and not covered by insurance so pure profit to the hospital.  The numbers are pretty low and the expertise required is high so I really can't see this being a preferred return on investment.  It's not a loss leader to be sure and last year it was estimated to bring in 600+ million dollars BUT, roughly 90+% was breast surgery, either implant or reduction/mastectomy.  By way of comparison, cosmetic implant surgery is on the order of 12 billion dollars.  I would argue that the surgeons who can do these procedures could generate the same profit margin from many far easier procedures.  I had the opportunity to talk to a couple of these butchers at a conerence pre-Covid because I was genuinely curious in the detail and although its butchery, the level of fine neural work and advanced graft techniques means I could learn something useful.  Both did this not for profit because as cosmetic physicians their general work was boring, this was engaging and challenging for them.  

I wish this could be chalked up to greedy hospitals but honestly, it can't be.  It's like blaming concentration camp guards for Nazism.  The cultural and political monsters are driving the downstream "need" to fill.

Note that I refuse to call them surgeons because while there is at least a debate as to whether its ethical for an adult there just isn't one to do this mutilation to children.  It's not just unethical, its antiethical.

I could not find the link now, but there was some small scandal related to a Boston transitioning division of a hospital, whose director stated that, bar some advanced cancer patients, long term treatments of transsexual patients, including following surgical procedures, long therm pharmaceutic treatments, and continued psychiatric support, were among the most expensive patients a hospital could find.

Of course, I do agree very much with you on the essence of the question.
